Hi!

Can someone tell me why OPEN REDUCTION is not preferred in SUPRACONDYLAR FRACTURE OF HUMERUS in children? :-

a) Fear of vascular and nreve injury.

b) Myositis ossificans traumatica.

c) Permanent stiffness of the elbow.

d) Infection.

You need to give more information that "Supercondylar Fracture of the Humerus" The indications of operative vs. non- operative treatment follow guidlines that involve, but not exclusive to, the fracture pattern, the location of the fracture, the displacement/angulation of the fracture, and the age of the child ect. If you are looking for sources look at "Rockwood and Wilkin's Fractures in Children". There is a nice chapter on that in there somewhere.

The fracture in this case was type II supracondylar fracture i.e. fracture has angulated poseriorly but periosteum was intact with no displacement or overlap. The boy was 8 year old.

Actually orthopod resident performed closed reduction with percutaneous pinning. I didnt had the opportunity to ask resident to explain since he was very busy. But to me it all appeared to be like solving a jigsaw puzzle in dark. Why didnt he just open up? Isnt it an easier treatment modality because fracture is obvious to all of us to see and decide whatever mechanical procedure is necessary?
