I matched in a very solid clinical/academic program starting next year. But I am less enthused on the location. Does anyone have insights r/ how difficult/rare it is to transfer programs? Are vacancies made public outside of sfmatch?

Very rare. Almost impossible, particularly if you're trying to find a PGY-2 (R1) spot.

If you're hoping for a better location, then it's even less likely b/c how many residents in a "great location" will decide to quit? Unlikely.

I know many professors who think that matched applicants who want to switch deserve not to match/transfer to a different program. Why is this? When applicants drop a program after the match, this literally messes it up for your future classmates (call, patient load, responsibilities, etc...) and the program. The program that loses you will now have to settle for an applicant that they likely would not consider before the match.

My advice to all the current applicants: don't rank any program that does not fit your personal and professional objectives.

Vacancies are announced outside of the match. Few are available, however.

Look here:

http://www.sfmatch.org/vacancies/r_ophthalmology.html#pgy-2_2003

There are currently no positions for PGY-2 in 2003 or 2004.

For PGY-3/PGY-4:

Eligibility:
These positions require prior completion of one or more years of ophthalmology training in an accredited program.

How to Apply:
Filled outside the match. Contact programs directly for application criteria if none specified. If asked to forward a copy of your CAS file, applicants may request that we forward our copy to a program at a cost of $35 per distribution. Applicants must also negotiate directly with the programs to determine eligibility, to be invited for an interview, and to sign a contract.
