I've counseled a few people before they've purchased. These were the smart people who wanted to ask a pharmacist before they bought the latest miracle weight loss drug. No one has bought it after I've explained how it works and what happens when dietary fat stays in your intestinal tract.
The marketing campaign for this stuff is ridiculous. They even have their own self help book out featuring their product. 🙄
We just got it in at Target. Can't wait for the customers to start rolling in. Haha, no pun intended.

The day before it came out people were calling like mad. I think not randomly defecating on yourself is a good incentive to watch what you eat.
On a side note, did anyone recieve the pound of fat from alli? lol. Pretty sweet. We had a floater that forced me to "take it down and put it somewhere" because she got sick to her stomach when she would look at it. In my opinion it ranks right up there with the stuffed enema toy.

Hopefully the fear of shatting themselves if they eat a fatty meal is enough incentive for them to finally lose weight
No customers asked me about it last night. 🙁 I was ready and really excited to discuss the nuances of anal leakage. From what I could tell from the fancy display, we had only sold 1 kit so far. I thought it was be a hot seller since there are soooo many fatties here in central Massachusetts and all the restaurants only serve greasy and/or fried foods. Maybe the word got around, though...
I work in a rich area Walgreens, and I had Thursday off. When I came in on Friday, it was all gone. Luckily we received a shipment of it late Friday. Someone did ask me about it, but it was before we received the shipment, and so, I did not get to discuss the "joys" of fatty stool.
I discussed it with one patient while we were waiting for her script (unusually slow night). I told her all about how it work and what she might expect. Her reply was, "I"m intrigued!" I guess scat never goes out of style.

Funny situation. Totally unpopular prescription medication goes OTC and is widely sought after. Usually its the other way around. Customer says "Oh Claritin used to work for me but since its OTC now Clarinex is all that works."

I understand why everyone thinks it's a bad idea but I personally think it's a good idea. People come into the pharmacy all the time to buy Xantrex and Trimspa or their favorite fat-burner of the week. Why not actually give them something that's FDA approved? Yeah the side-effects suck but that hopefully will help keep them from eating high fat meals (at least after the first one they'll know why). No it's not the ideal drug but at least it's FDA approved and the amount of money that people are spending will at least go to something where they know what they're getting in each capsule. Obviously weight loss should come from more than just a pill but hopefully this will help people make lifestyle changes. I just don't understand why everyone's so up in arms over this when everyday a new OTC herbal combo comes out claiming to help you lose weight and people flock to the pharmacy to buy them.
I don't necessarily think everyone is "up in arms" over this issue, it actually appears that people on this thread are discussing how entertaining the release of this product has been.
For me, I would have to say that I just think people are going to be very disappointed with the results they get. After a year of use, at $50-75 per month, the data indicate a 2 kilogram benefit when compared to just diet alone. Personally, I could find better things to do with my $900, and I also believe a good portion of the individuals buying this product are not really grasping what it takes to successfully get its "benefits." Do these patients know they need to take it for at least 3 months before you really notice any divergence in weight loss curves? Do they know it is not some wonder drug, and really has never performed terribly better than dieting alone? Do they know that most of the studies only looked at patients with a BMI over 30 (36 in the large trial published in JAMA in 1999)? Do they know what their BMI is? And, of course I have to mention the gastrointestinal adverse even profile; 25% report flatulence with discharge, I think that is the scientific way to put that 1 in 4 will report episodes of sharting.
I think the safety profile is well established, so I guess the best thing to do is let people have at it for a while. However, this is also a good place for you Pharmacists to jump in and have these discussions with your patients, and it sounds like some of you are. This is another medication that should probably be "behind the counter" so it would allow retail Pharmacists to assess its appropriateness for use, as well as make little progress notes on the individuals who purchase it documenting effectiveness, adverse events, discontinuation, etc. It would also be nice to know what percentage of people buying it from your store are actually regular customers at that particular pharmacy.

True, but I think you have to look at who the main consumers of Alli will be. These are the folks who are too lazy and unmotivated to do what really needs to be done to lose weight--exercise and eating right. I think they were probably too lazy and unmotivated to get their fat butt in their car and go to the doctor to get a Xenical script. But, now that they're already at the drugstore to buy the candy bars that are on sale, they can pick up some Alli.

I work at a Walmart in NC. I always ask the people if they have tried diet and exercise before. Usually they say yeah, but i cant stay on the diet. HELLO YOU HAVE TO EXERCISE AND DIET WHILE ON ALLI!!!! I am actually on pins and needles waiting for the ranting from the side efffects to come in. I also suggested to the pharmacy manager to put up a display of Depends right beside the Alli. Hey why not boost sales, they should come as a paired product with new dosing directions. "take one tablet by mouth and one depends undergarment topically after each fat containing meal"
i had a person buy one via drive-thru. Come on, you are going to buy a dieting pill that you need to exercise and eat healthy from your car, are people really that fat and lazy? You got a hot date that you can't at least burn a couple calories going into the store for a dieting pill? You are already destined to fail, after of course you have happy stool. When are people going to figure out that there is no quick fix.

I actually took one personally with a pepperoni roll as an experiment (It's a WV delicacy.....just think grease). It's really not that bad. No diarrhea yet...just frequent bowel movements. I could totally use this stuff to worsen my diet and not gain any weight.

It's an acne medication that is an analogue of Vitamin A. As such, it is highly lipid-soluble....which means Alli...being that it binds to fat....****s up how well it is absorbed into the blood stream.

When I took it back in high school I kinda went crazy. Well, more crazy anyway. I was depressed as hell.
Twelve years after I took Accutane I still have chapped lips! I should have bought stock in a lip balm company. It really did work wonders on my acne though, I have very few scars and people are surprised when I mention I had terrible acne as a teenager. This was all before the depression effects came to light...but I coped ok.
I've already heard Alli stories floating around my pharmacy. "Did you hear about so-and-so and how they went in their pants?" Craziness.
Come on now, if you were willing to do this experiment you gotta eat more than just one pepperoni roll. Try eating half of a deep dish meat pizza, then washing one down with a shot of cream, then finishing off the rest of the pizza. At least there is clinical evidence that Orlistat actually works as a weight loss drug, as opposed to Hydroxycut and all the copycats that are nothing but Ginseng, Vit C, and Ginko extract or some such BS. Of course if you read the clinical trial data you'll find that people dieting and on Orlistat only lost about 2.5 lbs over one year compared to people dieting who were not on Orlistat. How much did the average person dieting and not on Orlistat lose? About 0.1 lbs or something...so yeah, statistically speaking Orlistat is pretty effective over dieting alone...cause most Americans can't stick to a diet. I think that's where the whole "A pound of fat is more than you think", slogan comes in...at $50 a month for Alli, it's costing about $250 to lose that pound of fat!
24 hour fitness better watch out, might be some leakage going on when people are maxing out on the bench or squatting. Depends don't always work well with gym shorts.
Before Hydroxycut was reformulated it would cause you to lose much more weight, but of course, increase your risk of heart attack. Speeding up that resting heart rate tends to do that, which the ephedrine would do. I took that crap for like 3 weeks, and it made me sweat while wearing anti-persp. deodorant and not really moving much.
I still think the "Survivor" diet is the best diet, and as soon as I get on the same page as Mark Burnett, I"m going to get rich. No one on that show has ever gained weight and most have lost tons of pounds. The key: stop eating!

Recently, my former sister-in-law was contemplating getting a lap-band. Of course being the natural S.O.B. that I am, I told her she was a fool, to "quit f***ing eating", and that if she didn't the lap-band wasn't going to help one bit. Well, she had the surgery, she didn't quit eating, and she is 5lbs heavier than before the operation.

Accidentally sitting in leakage that people refuse to clean up is my primary concern. It's unfortunate that nobody takes responsibility for anything they do.
there is a guy that i work with at my other job. He weighed in at a nice 341 in november of this past year. He was put on Lipitor because his cholesterol numbers were like my high score on Tetris. Of course he got a nice warning from his doctor, but after that he put himself on a healthy diet which meant eating right and exercise. Since November, he has lost over 100 lbs. His cholesterol numbers too have dropped off.
It really is incredible to see that if people change their habits, miracles can happen. Now I know its not the answer for everybody and the medication helped, but how many people can say they put that much effort into it. When I see people buying this Alli, I wonder if they are going to make a similar decision of eating healthy and exercise.

Why have they watered down the strength? When you get an RX for Xenical it's 120mg while Alli is 60mg. This just makes me think that Alli isn't really that safe if they can't let the full strength of it out on the market. Or maybe they fear people will OD on it???

None of it is absorbed. It's the entire point behind the medication. I doubt people garner any toxicity from it other that GI troubles.
The reason the 60mg is out is likely so that the 120mg strength remains Rx only and they can continue getting the theoretical insurance revenue out of the stuff on top of the OTC sales. They did the same thing with Pepcid.
Stay away from the 400mg Ibuprofen...its dangerous stuff!
According to one of the many many, drug reps that we saw in June, the 60mg Orlistat has 75% efficacy of the 120mg orlistat. So if you avoid absorbing 10g of fat with the rx dose, you'd poop out 7.5g of fat on the 60mg dose. Or so said the drug rep with the low cut shirt and died blonde hair.
I can't wait for people to start suing because their clothing, or worse, self-esteem was ruined by a public BM. I gained 25 pounds last year. I went to the gym and started eating right. The 25 pounds is gone. (Actually 15 is gone and the other 10 pounds of fat replaced with muscle, go me!)
I skipped the drug therapies, so my BMs are normal. It really isn't that difficult!
