I'm confused...are you a 4th year now or are you an intern?
If you are a 4th year now, I would suggest staying out of psych and trying to either get into an ER program this year, or 2nd choice would be doing a medicine prelim year and then trying to switch to ER later.

If your board scores and grades pretty good for an ER applicant, and you can get an LOR or two from an ER doc, I think you should be able to get yourself a spot...perhaps not this year, but next.

Assuming you are a 4th year student, I think you should go to the ER dept. @your teaching hospital and talk to the ER dept. chair and/or program director, tell them you've had a change of heart, and ask for their advice about what to do at this point in the year.

Don't do psych if that's not really what you want to do, b/c it will make you miserable. I'm sure the psych programs would rather not train someone who is going to bail after 1 year, either.

If your home program is likely to have prelim spots in the scramble, then they would likely be very interested in ranking another quality applicant on their list. Ignore whatever deadline they have published. Contact the IM PD (or prelim PD if there is a separate person) at your home program and see if you can be interviewed. I do this all the time.

Same about your own school's EM program. I;m sure their deadline for application has passed. Still, they are very likely to consider an internal candidate even at this late date. You should talk to the EM PD also.

You can also contact other ED programs. State, very clearly, that you are a US grad who has changed their mind late in the game. You never know.

2. Time away from clinical medicine leads programs to worry that you won't be a prepared to start internship.

In my experience, this has been very true. I am far away from my medical school, so I can't very well camp out in the Dean's office in order to get the ERAS lady to take care of my LORs and associated documents.

And I can't count how many times I considered flying down there to do just that.

Some of us have 4th years that would probably make your toes curl. You know those med schools known for making you work insanely hard because the work hour rules only apply to residents?

Seriously, there is a small hole in the system that could help the OP. You're absolutely required to go through the NRMP match-- but you don't necessarily have to go through ERAS. So, you could theoretically talk a program into looking at your application outside or ERAS, if you can get your letter-writers and dean to agree to send on the information. This probably involves a very good support network and somehow convincing programs that its worth getting in outside the traditional deadline. Still, it certainly can't hurt at this point.

Chick. I had one of those 4th years, myself. In fact, I'll wager mine was worse than yours will be. Maybe someday I'll get over it, but I don't see it happening anytime soon. :annoyed:
