UCLA personal history form

Hey guys,
I just received a postcard from UCLA (which I haven't yet seen, since it got sent to my home address) apparently asking for the Personal History Form...

does this mean my applications in review? or do they wait until this form is in to begin the review process? ...just wondering

this is their secondary, they wont review ur application until you send it in. the postcard has a url on it, so if you can find it, you can get started.
