Has anyone (PreDent or Dental Student) had any experience with the Disciplinary Action Request Form, meaning, any ideas what schools require these? I am wondering whether Dental Schools contact undergraduate institutions without our knowledge, or whether all schools who do it will send me a form to fill out besides the ADEA Application.

No, I don't, I am just trying to be as organized as possible with all these applications and I just found a few schools with extra forms, etc to fill out online. I am wondering if they send one to you or if I need to download it, or it it only happens at some schools. I want to make sure I don't miss anything during the process. It's a lot to stay up with.

I don't believe any schools require the form BEFORE an acceptance. However, once you are accepted, certain schools (don't know all of them) will require a form. But they will most likely let you know when the time comes in your acceptance packet, etc. However, if you want, just check the websites of each school you applied to and see exactly what they require on the website.
