As a result of family health related concerns, I was unable to properly study and focus within the time frame of my STEP one permit even with one extension. I had to reapply for a new scheduling permit and am taking the exam this April. Does this affect me very negatively in terms of residency down the road? I just didnt want to take the exam and risk failing.

Thanks just real worried about that.

I don't think that it will have any effect
 
If you dont sit for the test, it does not appears in your transcripts, but for you it appears

test not schedule or did not present for the test.

If you do the extension, it does not matter either.

Yes yes I know who has the money to give for free to the NBME, but hey maquiavelo said: the end justify the means🙄
