Can anyone recommend an US-guided regional anesthesia course? I am looking for something that ideally has a cadaver lab where you can actually practice blocks rather than just scanning.

Full disclosure: I’m EM and mostly proficient in forearm/ankle blocks and fascia iliaca blocks.

No what he means is if u aren’t getting compensation out of it. It’s not worth the trouble as an ER docs

Just like anesthesia is asked to do lumbar puncture and collect CSF fluid occasionally. I refuse to do them.

My colleague has gotten sued as anesthesia for LP many years ago due to bleeding.

Don’t do anything that is out of ur scope. It’s fun to learn always but not practical in real life
The only person who benefits is the company u work for unless u do ur own billing which is getting rare even for EM docs (yeah there are a few private places still left like anesthesia)

We do our own billing and collections and would be compensated for the procedure. We’re also credentialed to perform them in general. Certain things like fascia iliaca blocks are likely to become almost mandatory for us to do depending on proposed CMS measures.

I mean a fascia iliaca Block is essentially a fem nerve block unless u do SIFI

For the turbo-obese I always just do infrainguinal and then put them in a little bit of Trendelelnburg and have a tech massage it upwards.

And what's your success rate with that? In terms of hitting fem, lfcn, and obturator? Probably quite low.

Yes. Just do SIFI. It is a very superficial block and easy to do.

That said, an infrainguinal fascia iliaca block=femoral nerve block will likely provide some rest pain relief for hip fracture patients in the ER. The problem is that potato chip little old ladies can’t get a subsequent high volume SIFI when they come up to the OR for their ORIF.
