Is it true that you must have passed both CK and CS to start residency? What do people do if they take the CS toward the end of their 4th year and won't get the score back in time?

There is no universal rule - it is residency-dependent. But, most residencies (that I know of) require that you pass both - CK at the very least. You will have to check with individual residencies. I think they list this requirement on FREIDA. My school required us to have taken CS by January for graduation.
 
I think this might be more a requirement of your school (i.e. you can't graduate unless you've completed Step II) than the residency.

I know this has been talked about around here somewhere ...
 
As I understand it, to graduate from medical school you must have passed CK and taken CS. So, in theory, you could fail CS and still go on to residency - just as long as you passed during your internship so that you could be licensed. There was a rumor that they were going to change it so that you had to pass CS as well, but then again, there was also a rumor that they were going to do away with CS.
