Hey All,

I need a little discussion on this Uworld question: So it is showing S. Na = 138, S. K= 5.6, S. CL=104, HCO3=24, Ca=9.5, Creatinine=1... the patient is on Lisinopril what is the other medication she is taking? the answer is Amiloride..... which make sense if we look only at the S. K ..... but isn't the S.Na too high for a patient taking ACE inhibitor and blocker of epithelial Na channels ..... any thoughts is appreciated...

You are right if you are thinking Amiloride might cause hyponatremia. It might. However this patient is not having that problem. The vignette indicates Na+ levels are WNL. Their problem is elevated K+. The question is testing your knowledge of Hyperkalemia in a patient taking ACEI. You got distracted. Dont get distracted.

Count on the examiners throwing tons of chaff at you. Dont go for them.
