Your best chances are your PA schools. Rutgers accepts very few non residents with no connection to the state. Apply more broadly and consider all these schools:
Vermont
Quinnipiac
Tufts
New York Medical College
Hofstra
Einstein
George Washington
Georgetown
NOVA MD
Oakland Beaumont
Western Michigan
Medical College Wisconsin
Rosalind Franklin
St. Louis
Creighton
Tulane
 
Your SMP GPA may hurt a little at some schools. You have a good undergraduate GPA and an excellent MCAT though and those are more important.
 
Why is that? SMPs are usually more similar to what one will experience in med school, but not nearly as rigorous. I've been told anything under 3.5 at an SMP is essentially DOA for MD schools as you have essentially failed your audition.

Well, if it's something that was completed three years ago and you have retaken your mcat, is it really fair to assume that?
