The one thing I am worried about is my sophomore year. I was struggling with family issues (not to make excuses) and ended up failing several classes. I do have two years of straight 4.0 but I'm worried that will be a huge red flag.

As long as the overall gpa is what you have described, and you have the single MCAT, you are fine.

When you apply include all your Michigan schools: Central Michigan, Michigan State, Wayne State, U Michigan, Oakland Beaumont and Western Michigan. Also include Toledo since they take half their non resident from Michigan.
