Been doing some inpatient acute rehab coverage as a side gig and as an independent contractor (somewhat crappy per diem for call and I bill professional fees). After months of run around, my biller told me that since I do not have a physical address (office) at the rehab facility I have to bill under the full time rehab guy and have him cut me a check like a locum. My medicare info is linked to my group practice down the road.

The regular rehab doc is solid/trustworthy but also a independent contractor. If my professional collections are billed under him and he cuts me a check I would assume we both get hammered with taxes (unless he deducts his taxes and I don't report his checks to me to IRS). We both want to do things above the board.

Is my biller cracked out? Are my assumptions on taxes correct? Appreciate any comments.

your biller is nuts. Since you have staff privileges at the hospital, the location of service is the rehab hospital. Of course you need to use the inpatient codes for your visits.

The other way could work as well, all he has to do is 1099 you for it. He could deduct 100% of his tax hit, so in the end only you would be paying taxes on the collections.

Thanks--been trying to figure this out for a while... 🙂
