what am I missing

I am a nurse caring for patient Will Smith. I note the admitting resident has documented the patient as having 3+ pitting edema of the lower extremities, dyspnea, cyanosis, and generalized chest pain.
I assess the patient and note via palpation, diffuse abdominal pain, hyperactive bowel sounds, and tachycardia. I double check the patient's identify with the arm band, his date of birth (DOB), and his medical record number. I ask the patient his name and DOB.

I call the resident to share my findings. What do you think happened in this case? Assuming the nurse is correct, what do the symptoms of Will Smith seem to indicate? What other symptoms would you suspect he is having? Diagnosis?

It's a lesson in HIPAA. Note that the nurse reveals the patient's name is Will Smith. This is a HIPAA violation.

However, let us also notice what the nurse did right. Will Smith is a very recognizable celebrity, but the nurse does not rely on this to identify the patient. She carefully goes and confirms that his wristband and verbal response indicate he is indeed the former Fresh Prince.

TL;DR HIPAA violations bad. Meticulous patient identification good.
 
Maybe his dictation got routed to the wrong patient or something.
 
Does one of the answers involve misdiagnosis due to palpating bowel sounds? 😀
