I couldn't find anything on their website, but a lot of people have been talking about it. Does it have to do with the Ddx and providing explanations?

Yeah, the orientation video they show you on the exam day is available to view ahead of time on the CS website. It shows you what the patient note will look like and even has a practice version you can type on to practice.
I think the big difference from previous years is that each DDx has place to list things from the History and Physical that support that diagnosis. The sample notes they have online to look at only list one History and one Physical finding for each of the two DDxs.
I would suggest going to the website to look at this stuff. It made me feel a lot better because the notes weren't like masterful prose or anything. Pretty basic stuff.
