Usually, you're responsible for making all travel arrangements, from airplane to interview door (including your cab ride and hotel). Many schools can give you advice on shuttle services or public transport, which can save you a signficant amount of money. Other schools have medical students you can stay with for one night, usually at a cheaper rate than a hotel.
The interviews I've been to range from super comfortable & friendly to extremely strange -- just go with the flow, stay calm, and recognize that there are some things, like significant pathology on the part of your interviewer, that you can do little about.
The interview will be in a doctor's office, hospital, or administrative office (although I've heard of a few that occured 'over lunch' or something like that).
Some places will plan an entire interview day, including lunch and a tour. Others will schedule your interviews and leave you to fend for yourself the rest of the time. A few will keep you in a room with several other very nervous interviewees for hours on end -- which can be quite Kafkaesque.
