When I was in UG I got caught taking a small amount of food from a dining hall. I had to go talk to the dean but he was basically just like, "idc but don't do it again" and I never heard anything about it after that.

Is this something I need to report? Is a scolding with no other consequences considered an IA?

An institutional action is a disciplinary action that goes on your permanent record and transcript. You would know if you have one.

It can be an institutional action that needs to be reported even it is not on your permanent record and transcript.

A verbal warning would not be an institutional action, IMHO. If you got "written up" or if you were put on probation or forced out of university housing due to a violation, that would be an institutional action.

You can always ask student affairs, registrar or your pre-med advisor just to be sure but I am pretty sure this is not an IA.
