What if the scalpel slips?

How common is it for a surgeon to make an error? Can these usually be fixed without consequence? I can't imagine a surgeon never making a mistake. What if a neurosurgeon accidentally cuts something or drops a blade and permanently disables the patient? Does malpractice cover these things?
