What is my LOR officially supposed to include?

It should be written on official letterhead stationary...and what else?

Is the doctor supposed to write my AAMC ID on there somwhere or anything else?

No aamc ID needed.

Key Phrases:

"I am delighted to provide a letter of recommendation for _____."

"____ was in my _____class from ___ to ___"
or "____ worked in my lab from ____to___ in the position of _____"

"As a _____ of over ___ years, I believe I can provide an accurate and candid evaluation of ___'s work."


List your role or what you achieved during the time they knew you.

Provide evidence of your work ethic, interpersonal skills, intelligence through 1-3 stories.


If they really like you, they'll throw in: "____ was in the top ___% of students I have ever taught." or "___ performed a level known of graduate students."

End with:

"I highly recommend ____ without reservations. As a nationally ranked medical school such as yours, your staff would be lucky to work with such a bright and capable young student such as ____. I hope you thoroughly consider ____'s application as you make your decisions for the incoming class; I ensure you will not be disappointed. If you have any further questions, please contact me at _____."

A truly good rec depends on the stories.
 
Make sure it includes a massive amount of clipart. People won't take it seriously unless there's an anthropomorphic toaster with an exclamation point over it. 🙂

The above looks pretty solid. Depending on the writer though, I wouldn't over-instruct him. If the writer knows what he's doing and has written letters before, he's going to be able to write something good that won't follow the form of every other letter, which is going to make for something that stands out and is more compelling overall.

That said, if the writer is not as experienced, it might not hurt to direct him toward a sample to look at.
