What kind of grade is consider a bad grade and needs an explanation?

Hi,

I just came across with a question with explaining bad grades from the past on a secondary application. So I just want to throw this questions out there. From the adcom perspective, what letter grade is warranted an explanation?

They are looking for something substantial like debilitating conditions, accidents, death in the family and not the more common-too many wine coolers, hangover/ party city.

I would only address failing grades, ie. D's or F's...C's aren't great either if you've got a few, but they're considered passing so I don't see the need to explain.
