what would you do when your patient dont like you ?

They are probably just tired of answering the same questions from med students especially if they are "bouncebacks" to the service. Crack a joke with them and see what happens. Sometimes they'll open up to you then.
 
I had this happen to me once in the ER.

I told the chick that if she didn't start talking and stop crying/bitching/moaning that I'd have her thrown out of the hospital because she was refusing care. There are plenty of people in the waiting room, I said. She started talking after that.

Too bad for her she had IE and had to stay; of course, that didn't stop her from bailing five days into her admission with her IV line dangling away, ready to go and shoot up on a street corner somewhere.

Just know that their projection is their current state and has nothing to do with you. I've been in dozens of situations where the patient just didn't feel like talking and initially, I pushed back hard (Then why are you even here?). Eventually, I learned that small talk in the beginning, changing the subject, asking about certain non-medical things you notice works wonders. Only once have I ended an interview because I just wasn't getting anywhere. He was a homeless guy for EtOH detox and kept saying that he just wanted to sleep and I was like, go to a hotel. But after his nap, we were able to get down to business no problem.
