Also can you do a the flight surgery or UMO course? Seems like fun and a good break from regular medicine

You can apply during 4th year, but they probably won't accept you until after you actually start. Specialty will impact possible acceptance. Can you do FS or UMO? Yes. You would have to work that out with the detailer. They would likely want you to do it after your first utilization tour.
