When should you send a letter of intent to attend a particular medical school? With the secondary?, after the interview? This is a long way off for me, but I like to know the long and short of a situation way in advance usually

It's not a formal part of the process. There aren't any rules for it. Perhaps you should actually go to the school's interview before pledging to absolutely turn down any other school, no matter how much you like it. I'm still really against these letters.
