Which schools have a certain form for LORs?

Hi, everyone-just a quick question. Do AZCOM, OSUCOM, NOVA, or UHSCOM require you to use a certain form when submitting your LORs? It seems like I remember hearing something about how some schools do this, but I can't remember the details. Thanks!

the ones that i applied to said that their forms are optional, especially for the non-traditional students who have been out of undergrad for such a long period, and that you can use your school's premed committee's form.

AZ - secondary consist of a photocopied (tacky green) plain paper with no other forms

I think that a lot of them have their own form. I used only the pre-med committee forms from my undergraduate school. They are handling all my forms and sending them to the schools I request them to be sent. I am glad they are doing this even though I graduated last year. No school has objected to the generic forms. Could you imagine if your LOR writers had to fill out a new form for each of your schools. It would be about like us filling out new secondaries for each school. And we all know how much fun that is.😀

Just to add my .02 I used my premed committee for my undergaduate LORs and I also just sent in my DO LOR without a form either. I did not use any of the forms that anyone sent me beacuse I felt it was too much to ask the people that were writting my LOR to do a seperate sheet each time. I had no problems at all without using the forms. The only problem I did have was Nova not getting my UG LOR the first time I sent them. 🙄

Nova has a form. I'm mad I can't find the stupid thing!
They send you four, but I think I gave them all to teachers and forgot to save one for a DO.

I'm having a Homer moment...DOh!

I will have to do the same, especially that one of my sci prof hasn't turned in a lor at the premed office (although i have 2 other sci lors, this prof actually knows me really well and i know it would be a good lor) despite my frequent reminders and even meeting him in person. I had asked him in mid-July and it is getting quite frustrating because the secondaries are piling up ( I have 2 midterms this week so well this coming weekend is going to be a lets- fill -out -the- secondaries-weekend). Anyhow back to the point, since my prof are already taking this much time (and they better finish it this week because I just sent them reminders this past weekend), I will just have to call up admissions and ask if the general forms from my undergrad are ok or not.



On an offnote, I want to ask you all if you feel that your personality has gone/is going through a major change? I feel that I am getting so impatient lately with everyone, am stressed, and can almost feel the pessimism set in. I used to be a calm, patient, happy, optimistic person just few months ago. ARgh! I feel as if this process is going to make a monster out of me. Ok, i am exaggerating but I can feel myself changing, i just hope it is temporary. I usually could be calm with running but at this rate, I would be running all day and still feel this way!

Oh, forgot to mention. It is VERY IMPORTANT.... any form you do have your LOR writers send... Make sure you have waived your rights to see the letter and have SIGNED it.

All the schools have that requirment on their form.

I applied to ten schools: AZCOM, CCOM, NSUCOM, NYCOM, PCOM, Western/COMP, OS-COM, MSUCOM, TUCOM, and UHS.

Of these ten schools MSUCOM specifically states in their literature that you MUST use their LOR form and you can only have two. Any other recommendation letters without the two forms will not be included in your file. I sent my school's premedical commitee packet (cover letter from school plus copies of all of my LORs) to the other nine.
