You should read my whole post. I specifically said the forensic units at state hospital are not good.

The state hospitals in my state are terrible in their entirety, not just forensics units, due to low funding (low staffing ratio, poor training, poor retention).

One of my students told me once that they liked OB/Gyn because the patients weren't sick (as in maternity).

In all my rotations and research, the most compliant patients were IB/GYN. It is a big reason why I am picking the specialty. Patients are young, trying to do their best. The older patients are willing to work to beat infertility, cancer, etc. Also, those who have had chronic pain are thankful to have someone take it seriously.

And probably the most Dr. Google patient specialty of all of them. Every patient thinks the internet is more knowledgeable than you and can't be wrong that eating watermelon is now bad for pregnancy because blog 47 said so lol.

You are obviously looking for patients who show gratitude. With our current reimbursal situation and the fact that you can longer count on medicine to make you rich, this is a fair goal. I am 66 y.o, and it is patient gratitude that keeps me going as an ophthalmologist.
My advice is to steer away from specialties that make your job dependent on someone else's performance (like ER staffing). Also stay away from chronic illnesses and self inflicted disease.........do you think it is fun to tell every patient to lose weight, quit smoking, and avoid stress?
If you start recommending Prozac for every patient, won't you at some point think you may also need Prozac?

I have to second vascular as being generally frustrating. Any of the various pain specialists will also see a very certain subset of the population.

As far as biggest, most rapid change from often unbearable to extremely pleasant?

My mom had back labor with my older brother. She had already been given something that made her a little bit loopy. The anesthesiologist had later placed her epidural and she had said him, "That feels so much better I could just kiss you. Let me kiss you." It wasn't creepy like it sounds on the Internet because my mom is just the sweetest lady who was clearly a bit out of it. It was funny. We still laugh about it in 30 years. 😛
