It seems as if a surgeon would not be able to practice nearly as long as a dermatologist. Simply because of the decrease in dexterity as one gets older. Is this for the most part true?
The late DeBakey was operating well into his 80s. The "dexterity" required for surgery is greatly played up -- most of what you use are technical skills that must be painstakingly learned.
Whatever small slips might occur in one's dexterity would likely be more than compensated for by one's massive experience and judgement.
