I was also initially concerned some may attempt to find fault as if there was a reward for it. Im glad I was wrong, the exercise was helpful.
Jon Snow said:Yeah, but in this scenario, why would we need the psychiatrist?
Patient goes to psychologist, psychologist refers to PCP, PCP orders lab to run tests. Done deal. Same result.

Jon Snow said:This suggests to me that your opinion is that psychologists are not competent and not inclined to consider anything but "inorganic" causes. Ergo, you don't need a psychologist for diagnosis.
As has been illustrated by those psychologists that have posted in this thread, that is not the case. Basically, this is the exact same point that I made in saying, "What do you need a psychiatrist for?"
Jon Snow said:If you say so. I had a psychotic patient who used to say that all the time. Fred, is that you?
Poety said:First of all: Amen Solid
Second: SAZI CAN YOU TAKE CARE OF THIS TOOL PLEASE?
EDIT: I have not had a problem with any other psychologist on this forum but YOU Jon Snow, despite trying to be fair and objective about the whole thing- I really do wish you would voluntarily leave this forum as you're no longer welcome here - and you're really starting to irritate the rest of us who make an attempt to get along with our peers.

Jon Snow said:Correct! You have good taste.
As far as Poety.. . . zen, my friend. You're out of balance.
Solideliquid said:Slippery slope. I am saying psychologists are not medically trained to handle Axis III problems. I'm not saying psychologists are not needed.
I agree, psychologists are not medically trained to treat Axis III problems. However, medical training will provide psychologists with the knowledge to know when to make appropriate referrals to other health care providers. Psychologists without medical training are far more dangerous.
I think SOME psychologists are trying to play physician, and taking a weekend course is not medical school.
Jon Snow said:Just out of curiosity. Explain the catastrophe of prescribing an anti-depressant to someone with hypothyroidism?
Anasazi23 said:Effexor is a bad choice in this case.
Effexor can exacerbate a thyroiditis, and generally shouldn't be prescribed in a patient with cardiac complaints, with exceptions.
PsychEval said:Thanks, I am genuinely interested in psychopharm and RxP training. Unfortunately, there is no prescribing psychologist message board.
Poety said:I have no idea what the realm of psychologist RxP covers
mosche said:I've been at work for the last 8 hours. I get home and I find you all fighting! That's it. No Pads for ANYONE! Now get to your rooms and don't talk back to me either. Someone's NOT getting a visit from the Christmas Bunny, and at this rate, the Easter Claus is highly questionable!
psisci said:Anasazi, I disagree. I already ruled out thyroiditis...guess why? Anyhow I would not in any reality attempt to treat this patient. I would refer back to GP with a note to look at labs, as I have done many times, and only Rx an antidepressant if Sx were still present post 6-12 months l-thyroxine...and only with GP approval.
Take it easy on Snow...he is opinionated and I wish we had more of this around and less of "You are right old buddy"
PublicHealth said:Daddy, please don't spank me!
As I type, Tabitha Stevens, a pornstar, is holding an egg between her buttcheeks and being robospanked on the Howard Stern Show.
Anasazi23 said:The pathogenesis of this patient's hypothyroidism hasn't been considered (look up pregnancy-induced hypothyroidism). With this you'll see that it is a thyroiditis that results in this condition due to autoantibodies developed during the pregnancy. She described the symptoms classically. This condition often results in a hypothyroid condition after resolution of the thyroiditis. It is a common pregnancy complication, and can sometimes result in permanent hypothyroidism with future risk for nodules.
While Effexor likely won't cause any serious harm, it has a small risk of exacerbating the hypothyroid condition, or putting her at further risk for goiter or nodule.
mosche said:BTW, is it a Christmas egg?
PublicHealth said:Nope. Grade A chicken eggs stuck between a Grade D pornstar's buttcheeks.
mosche said:I came back to this thread expecting the same old s___! Your post, however, made my day!!!

Get well soon Poety!
pschmom1 said:Poety, I've noticed that you have a new baby (cute by the way) and are in med school, Im just curious where you find the time to post all day long? I've read this entire forum because I found it quite interesting and humorous, and you posted for almost 9 hours in one day. You obviously have excellent time management skills or a nanny. By the way, don't let John Snow get you so stressed out. You said 4 or 5 times throughout your posts with him that you were done with him, yet everytime he posts you freak out. You obviously like the other posters here so simply do what you said you were going to do several times and be done with him. Just because you don't like him doesn't mean he can't post. You seem like you are rather intelligent and have interesting things to say but get so easily distracted and really upset about Snow. Just my 2 cents. Not trying to be critical, just hopeing to calm you down so the negativity can be dropped and good discussions can be had. Were all adults here . Thanks!
pschmom1 said:Poety, I am sorry that I've upset you. I didn't realize how harsh my thread was and for that I apologize. I have no bias towards Snow, and again I meant no harm. I hope there is no hard feelings . Thanks.
