I was wondering if anyone can recommend a good OMFS in the NJ area (zipcode - 07601). I am looking into getting this done. I have heard some horror stories about how the procedure can go wrong. It would be great if somoene could point me where to look or just recommend somoene close by to me. Thank you!!

Edit - Could someone tell me how long the recovery process is from this process? I am going to be starting clinical rotations Monday. I don't want to go in the first day high on vicadin with a swollen mouth 😱

are you saying that you don't know yet which OMS to go to, yet you want to have this done and recovered by monday? that probably won't happen because you need to set up a consultation and then schedule an appointment (some OMS do both on the same day)

usually my patients don't require narcotics for more than 1 day post op if that; however, each case and patient (pain threshold) is different. As far as swelling is concerned , that is VERY dependent on the case and patient. For normal partial bony impactions, I've noticed that the swelling is the maximum at about 24 hours post op and then gradually goes down over the next 3 days, and imperceptible after a week. Your OMS may give you IV steroids just before or during the case. This may reduce the edema at about the 2-3 day mark but won't make a difference at the 7 day mark. My recommendation is go in for a consultation, and if it's not an emergency, schedule your extractions during a break that you may have, or even right before a long weekend.

Thanks for that insight! I just scheduled a consultation with OMS for friday at 12. The lady on the phone said he may do it same day. The guy is in the "Best dentists in America" book. I just hope all goes well if the procedure is done.

It is interesting you recommend that I hold off until I have a weekend off. I wish I could do this in a three day weekend. I begin IM core on Monday and then I have Tuesday off. You mention IV steroids reducing swelling at the 3 day mark. Should I ask for this beforehand or just let the OMS decide? My only concern is looking like a tool on the first day!

most wisdom teeth cases are uncomplicated and there's no problem if he does it the very same day as the consultation. in fact, there was a study recently in the journal of oral and maxillofacial surgery that compared outcomes of extractions done with a separate consultation appointment and without. there was no difference so that's good.
i just mentioned the weekend thing incase its difficult to get off work. i recommend to my patients that they take atleast one day off to recover, especially since they may have to take some narcotics on post op day 1. one advantage of doing it on a thurs/friday is that if you do end up with a dry socket, it will manifest itself on mon/tues, and that way you can see your OMS. Otherwise if you end up with one over the weekend, you may have to wait until monday to see him. I would not ask him for IV steroids or even bring it up. Some OMS like to use it and some don't. Either way, even the difference i mentioned probably won't be all that significant for you to worry abouit it, even in terms of discomfort. personally, i look at post op edema to some extent as a healing mechanism, allowing the body to fight off infection by allowing your PMNs to extravasate, which the steroids will prevent. We get more concerned about edema when we operate around the airway. so don't even worry about the steroid issue and let the OMS do his thing.
 
is your wisdom teeth impacted? Those can get pretty messy. If not, it's a simple numb and pull extraction.
 
I had my left wisdoms extracted last week while at school and was back to seeing patients the next day. One of mine was impacted and the other had erupted. On hindsight I should have maybe taken it a little slower the following day but our grades for clinic were due. I had a lot of swelling but was told that happens to light skinned, light eyed, red hair patients. No bruising and not terrible pain. It is actually more uncomfortable more now a week and a half later. Good luck.
 
Scalpel - Thank you again for clearing up things. I think I am just going to ask for it to be pulled on Friday (if he gives me the option). That way, hopefully, I would be in decent shape for first day of rotations on Monday. If not, I have Tuesday off. That should definitely give me ample time to recover.

INFNITE - I do not know if my tooth is impacted. It just started hurting yesterday. It got to the point that I wanted to cry b/c I was in pain. A couple of advils took good care of the pain though. I am hoping it's not impacted and it's a simple pull. 🙂

Hmmm - how so is it more uncomfortable now than before? You mean its painful or...?

You gotta be kidding me. Don't be a sucker here.

What are the criteria to land in this book?
Number of adverse outcomes? Number of complications? Years of experience? Yearly income? Number of patients treated? Patient satisfaction? Shortest healing period? Least painful postoperative period? Highest number of referrals?

The fact is that these "lists" are media tools based on nothing but the opinions of a few people.

It hurts when I bite down. Nobody else I know had the same kind of pain though.
 
My wife and I had this done last year. I had no swelling or bruising, but she had both. The day after the procedure our pain did hurt quite a bit, but it was bearable enough and we took painkillers. We also had two days off. I did have a problem chewing on my inner cheek, so I had to use cotton rolls for about 3 or 4 days. I'm glad it's over with. Impacted wisdoms are nasty food traps!
 
I am going in today! Will update and tell ya how it went (if it goes at all).
