withheld info on criminal history

I have gotten myself into a rut over a misdemeanor about a year ago on New Years Eve, I was arrested for Public Intox, Disorderly Conduct and Resisting Arrest. I spent a night downtown and released on $600 bail. I plead guilty in court and paid the $500 fine hoping to get the whole ordeal put to rest. Since I was visiting the city where I was arrested, I did not have the money or time to get a lawyer and fight it. Although, they had no reason to charge me for public intox, since I refused the breathalyzer/blood/urine testing.

This is a very embarrassing situation, and I did not disclose it to my state medical licensing board on the application, which was foolish on my part. I have already received my state resident permit. I have a guilty conscience and greatly fear that they will dig up my criminal record and revoke my resident permit because I withheld information on this one incident. Should I be worried? If I tell them now that I lied, will my permit be revoked? Or should I keep it quiet and hope they they won't find out (this would drive me crazy)?

So you already have your medical license?

If so, I'd just let it go. Why would they do another/more thorough background check than the one they just did?

Yes, I have the resident permit from the state licensing board they sent it 2 days after I submitted my application, but I'm still concerned that this slip-up with the law may come out in the future. I'm not sure if they even did a background check since my resident permit arrived so quickly.

Immediately get professional counsel. I would call a criminal law specialist like now, err like yesterday. Yes, you can not only get a permenant revocation in your current state but it will also prevent you from getting one in other states, Florida with the recent amendments they passed being the most notable. I wouldnt tell the state med board but I would prepare if you are assaulted with this.
 
Why did you refuse the breathalyzer?
 
I agree with LADoc00; you certainly need to get that straightened out. Medical boards hate when docs lie to them, and they can certainly hold a grudge. Find a lawyer, and make it one who's dealt with medical registration issues.

By lying on the application, you turned a misdemenor into a felony (perjury). I would talk to a lawyer ASAP.
