so I'm a scribe and I put down where I worked, how many hours I did and all that. Then I started talking about everything I did as a scribe so for instance I wrote: I do most of the charting for physicians. This includes taking histories and writing HPIs, preforming ROSes, filling out physical exams, procedures, and differential diagnoses as they're dictated to me, assisting in discharge/admissions, and finally writing any progress/consult notes into chart as needed. I'm also a trainer scribe at the site that I work at. This means that I would preform chart audits to make sure that other scribes/trainees are filling out charts appropriately. I also train trainees in how to correctly create a chart and interact with physicians/providers.
