Worst Hours/Malignant Programs

OK, we have had a thread talking about the best residency hours........what about the worst ones. What programs are notoriously malignant hours-wise. Who works their residents harder than other programs. Any of you out there care to comment?
 
As far as long hours go, Stanford has 20-12 hour shifts a month, often with a long commute to boot. The hospitals are spread apart so that the commute can be about an hour north to SF and an hr south to Santa Clara from Palo Alto or the peninsula where most residents live. It's not malignant though, and shifts are not that busy at some sites. They're not worked to death, but spend a lot of time at work.
 
UMass had a rep for long hours but they went from 12 hour to 10 hour shifts this year. From what I understand it made a huge difference. (Added bonus new ED in two years).
 
At Lincoln in the Bronx, you work 20 12s at least your first year, and I believe you get one less shift per month as a junior and senior. You work hard, although I wouldn't describe it as malignant - just busy.
