AMCAS Questions - please help

1) I withdrew from a course so for course type I mark on withdrawal but for OT grade do I write "W" as the grade or do I leave it blank

2)Experiences section: for contact name do I write the name of the person I worked in with in Lab or the name of the principal investigator

3)What is the course classification for an Honors Research course. Also, do I mark honors for course type.

4) Marking incomplete on the course type, do I put an "I" for the OT grade.

5) For course classification, let's say you were an chemistry lab assistant so do you classify this under chemistry or education
