im confused about evaluation letter

it said that applicants are required to submit one health profession's committe evaluation packet...is it TMDSAS evaluation form filled out by pre dental advisor or is it a letter written by pre dental advisor??
 
It is a letter written by a pre-advisory committee. At my college it is a board and they review your letter of rec's and submit a final evaluation. My college has a form similar to TMDSAS, but I believe you have to send a release from TMDSAS (it's on the website) regardless, and they send it in. My college submitted electronically. If they haven't prepared anything you can print out the form to give to a couple professors and they can send it directly. Don't forget, you need one from a dentist for Baylor. The whole thing takes a while through a school committee b/c they interview you and prepare all of the stuff to submit. If you didn't go through that I would go straight to the prof's at this point. They could probably get it done in a week.
 
Both. You need to fill out the form, sign it yourself, and have the health prof committee sign it too... Now the committee has a choice to either just fill out the form, which kinda sucks because there isn't a lot of room to write great stuff about you, or they can submit a group letter or each a personal evaluation letter. It just depends on how well your committee does its job. Either way they ask for the form to be sent with the evaluation(s) so they can match it to your file.

Each school is different from what I hear. So it might be one person at one school or by committee at another.

At my school when I requested my letter of reference I submitted a list of previous professors who know me well, a letter explaining my motivation to enter dentistry and a formal written request for a letter of reference from the school. I included the TMDSAS form with this request and where they need to send the info. The Health Professions Advisor asked three of my previous professors to help write my letter(s). I was asked to come to an interview and I answered some basic questions about my goals, motivation and etc... Therefore, my letter(s) was submitted by comittee. From what I hear it is pretty common to have a comittee of past instructors write your letter. The point of the letter is to back up that you are a good student, so I think it is better to have your actual professors write your letter than just an advisor who might or might not know you well.

I keep saying letter(s) because I didn't see what was sent. I waived my right to review it. I am pretty sure that all three instructor wrote something, but I do not know if it was one letter or many.
