Application Withdrawal Etiquette

Not that I'm at this step at all yet, but I keep reading MDApps where these applicants withdrew their applications to numerous schools either because they couldn't afford to submit another secondary/attend an interview or they had already gotten enough offers from other places.

I understand withdrawing applications in cases where the application fee can be refunded, but what is the etiquette in situations where it can't? What's preventing these applicants to simply just ignore/not submit/not attend interviews for these other schools? When is it okay simply to ignore and when is notification necessary?
 
Even if you're not getting the fee back, it's a professional courtesy to let the program know that you are withdrawing your application. I think it's less important if you're withdrawing before even submitting a secondary, but scheduling an interview and deciding not to go without letting the school know is extremely poor form.
