Do you have to tailor your application to prelim spots from radiology to IM? for example, your letters say why you would be good for IM, and your personal statement.

No. My personal statement was all about Rads, and I did fine when it came to TY/prelim interviews.

The only thing I changed was including an IM chair letter for internal medicine prelims. It wasn’t necessary though; I just did it since my med school IM department made the chair letter really easy to get (just a 20 minute interview with an IM faculty early on in M4).

1 line at the end of your PS about how a ty/prelim/surg year will be a good foundation before advanced. if you really want to go to a specific city, mention how you have ties to the area and want to stay there for internship.
