It depends if you have your own practice or not. If you work for a big hospital or clinic, 40 hour weeks are normal. If you are a sole practitioner, the sky is the limit when it comes to hours. Most good ones work VERY HARD. You are always on-call to your patients 24/7. You never really get to go on vacation.
That being said, I can't imagine why people go into medicine just to work for someone else when they have such a marketable skill. But I'm pretty different from a lot of other people in this field as it turns out.
