Binocular Vision Text Recommendations?

Does anyone have any binocular vision textbooks that they recommend? I am looking for something that is fairly straightforward and discusses how to diagnose and manage accommodative/convergence dysfunctions (a book with case examples would be preferred).

I agree...this is an excellent, easy to understand/follow book with great case studies to help reinforce the concepts and place them into real world situations.
