you have two clocks one for the day and one for the block. When you take a break use your watch to know how much time is left. You get 45 minutes of breaks, but if you complete the tutorial early, that time is added to your break time. Go to the nbme website and go ahead and take the same tutorial that appears before the test on and you should have around an hour of break time. I took a 5 minute break between most sections and a 25 minute lunch, but you can use your hour (or 45 minutes if you do the tutorial the day of the exam) as you see fit, ie 10 minutes after 2 sections etc. Hope that helps.

If you click on 'time remaning' during the exam, the screen that pops up shows how much break time you have remaining in the day. If you finish a block early, that time is added to the break time. Conversely, if you run over on your break time, that time is subtracted from your final block.

Also, if you finish an exam section early, that time gets added to your break time. As a result, it's pretty easy to accumulate a lot of time for breaks, thankfully! :)

I didn't wear a watch and just kept a general idea of the time from the clock in the waiting room. From signing in and out and checking the computer in the exam room you know where you stand in terms of time.
