What do we need to do with transcript matching form in CAAPID application, as i already have sealed transcripts from my university and sealed LORs. Do i still need to fill the transcript matching form and LOR evaluation form?????????plz help

You can put them in a separate envelop and clip them with the LOR or the transcript. The form has a bar code that will enable them to match the documents with your application in case there is difference in the name at the documents and your application

at least somebody tell me that do i need to get TRANSCRIPT MATCHING FORM signed and/or stamped by my dental school registrar or just sign it and tie it with my ECE report and send to caapid???????????????????????????😕😕

Just print it and fill up the information of the evaluator, you do not need it to be signed by any one.. it is very easy and simple
