If you get Canadian electives in your desired specialty and due to program aren't able to get US electives but have a strong desire to match in the US. Will this hurt you with US programs (assuming you are able to get observerships in the US as well).
 
Hard to say. In general, Canadian rotations are supposed to be equivalent to US ones. Canadian schools are all LCME accredited, although residency accreditation is different. It will probably depend upon what you did, what field you're interested in, etc.

Lets say its in internal medicine, and they are basically equivalent to core rotations in medicine.

I am also wondering about how to get letters of recommendation from US docs if i don't do clinical electives in the US.

Do letters from Canadian doctors work?

If yo haven't done rotations in the US, then you're not getting US letters.

So, in that case, Canadian letters are all you'll have. Some programs may care that they are not US letters. Many / Most programs will not. It would be better if your Canadian letters were from one of the large programs that are more known in the US.
