changing diagnose ?

I want to ask resident doctors out there how often does a doctor change the diagnose for a patient.

Let's say a patient has a mental illness diagnose, do psychiatrists change the diagnose now and then?

I know a person with a mentally illness whose diagnose was changed a few times and her medication was adjusted because of the changed diagnose. I know mental illness does not have a lab test and I believe the doctor diagnose by observing the patient? what if a CT scan shows no sign of brain abnormality?
