sooo took my CS about 2 weeks ago and this has been bugging me... felt like i did alright on most patient encounters (im sure i missed some stuff but nothing too blatant) but the phone one threw me off entirely. I totally missed the diagnosis (which was so obvious the moment i started the note that it hurts to think about) so i missed a bunch of important questions in relation to the diagnosis. also i told the pt to come in but they had no access to a car at the time so i just said "come the first chance you get, i dont think its an emergency" instead of telling them to come in immediately... which may have been incorrect after realizing the correct diagnosis, and that seems like the most important part... basically i feel like i totally bombed this pt encounter so my question is, can bombing one encounter lead to a fail overall? also i know 2 encounters dont count, are they random or do they drop the lowest ones?

Firstly, ill state that noone knows if they drop the lowest encounters or just pre-determined random ones .
However what i can stay with a lot of certainty is that ''bombing'' one encounter does not fail you .
I took my cs in aug and i felt like i failed while totally bombing more than 2 encounters including one which resulted in possibly the most awkward situation in step 2 cs history (with a female patient) .
I passed while doing the most popular things right ; washed my hands , was pretty nice, told patients everything i was doing, summarized et al.
If you did most of these things most of the time , you're a sure bet to pass .
Don't worry about it. I did and i regretted it in the end . Go out have fun, get laid and relax.
Don't worry till you have to.

I wouldn't worry too much about it. You can't fail by tanking any one encounter. Pretty sure you could tell that Marfan's guy with 10/10 tearing chest pain radiating to the back that you'll see him in clinic next week and still pass if you do a competent job on the other cases.

I also found the phone interview very awkward, doubly so because there was pediatrics involved and don't really remember how babies work. It's fine.

I asked a guy with obvious pain like 5 times if I could examine him (it was my first patient and I was still really, really nervous and couldn't remember if I was supposed to accept his answer or explain why I needed to examine him).

I figure, even if that part of the checklist is a zero, I got most of the other stuff right. We say we "bombed" encounters- but it's usually 1 aspect (like one important differential, or forgetting to counsel on 1 aspect) that we forgot. Don't forget all of the things you did RIGHT!

That's what I keep telling myself anyway 😉

Dont' worry about it. My case was so ridiculously awkward it was hilarious, and I finished it with a good 6 or 7 minutes to spare. Still passed.
