CS Critical fail?

There are many threads here detailing the numerous mistakes (perceived or real) that people have made on CS, only to find that they pass.

My question is: what is actually considered a critical fail? Can you really fail the ENTIRE exam because of ONE screw up in one encounter? What would such a mistake be?

There are. But only 10 count toward the actual score. No one knows which two aren't. But they do that for experimental purposes. My guess would be if there's a phone encounter or something, that could be one of the ungraded ones.
I didn't have any phone encounters on my exam day. So like you said, no-one knows.
