How do you diagnose urosepsis?
Where is the best place to find this information?

Elderly patient with clinical signs of shock and the almighty mental status change.

Should be in any major clinical textbook. I always go with the Current series. Marino is fantastic as well.
 
How about positive leukocytes in the urine combined with clinicial signs of infection + Gram (-) rod cultured from the blood.
 
That would be uro-bacteremia. To truly diagnose urosepsis you need to have . . . sepsis. That said, "urosepsis" is one of the most misapplied diagnoses I have ever encountered. I also believe that it cannot be coded for with an ICD-9 code, instead they code for pyelo or UTI and sepsis.
