hey so question for an assignment. what are the requirements for discharging a patient from the hospital to say a nursing home or SNF?
been looking on the internet but can't really find anything

Requirements? To be able to be there without dying. When doing this assignment, think about the differences in monitoring and the amount of help/supervision each setting provides. (Could a patient who is disoriented but has several competent family members could potentially go home vs the same patient with no supportive family?)

You have to justify your recommendation. Like what's mentioned above, using your clinical judgment, why can't the patient go home? And what does the patient need?
