Not necessarily. A lot of older surgeons in my area look through the scope for all their sinus cases and don't use any camera/monitor system. I think it's a pretty good bet that whoever did this case trained in the pre-FESS era.

I thought about that, but even the angle of the scope and the debrider / whatever instrument is being used...
