Many of the secondaries allow one to choose between "regular MD" or a dual degree (MD/MBA, MD/MPH, etc). Does choosing "regular MD" bar one from later doing a dual degree at that school? Or is this a done deal?

This will also depend on the type of degree. For example, the MD/PhD program at my school will consider first and second year medical students for admission, but I've never heard of this happening for the MD/MPH program. This is probably because many of the MPH requirements are fulfilled during MS1/MS2, while the only PhD requirement completed during those years is the summer research rotation between MS1 and MS2, which can easily be made up later.

I imagine this works similarly at other schools. If you are interested in one program in particular, it wouldn't hurt to email the program coordinator and ask about internal transfers/applications just to be sure.

Depends on the school. My school's dual degree programs (MBA, MPH) were done after third year, took two(ish) years, and courses double counted for the second degree and fourth year requirements. You'd fulfill the remaining credits during the summers. So, our students didn't apply to the dual degree programs until M3.
