When listing my ECs on the med school app will I need to provide proof of my activities? I volunteered at several hospitals, will I need to provide proof of my hours for each?
 
You should list a contact person at the hospitals (volunteer coordinator or whatever) when possible. Though a school won't verify every single EC of every applicant, they might take the time to verify a couple of key ECs for candidates they're considering interviewing or accepting.

There is a spot on the app to provide contact information for your jobs ect. There's no rule saying you have to fill it out, but it is there. I would reccomend filling in the contact info though.
 
They can at least verify you existed. I doubt my hours for when I was working at a hospital with a user database using only 4-digit user IDs and 4-digit passwords still exists, but I am sure that I am listed as having worked there at one point...
 
You won't have to provide proof. It's an honor system.
 
They will require real-time video of you at any activity you list. VHS, digital, and film reels are accepted. Each frame must be signed by the contact person and notarized by a registered notary public.

Above have it right. For proof, all you'll need it the contact information for someone there. Sometimes you need to bend those rules a bit (ie, Mr. X died, but Mrs. Y worked in the office, so you'll put her instead - it's just simpler).
