EU student - residency in the UK?

Hello,
I am studying at an european med school and would like to do my residency in the uk. I'm wondering what the best route is:

-finish med school here -> foundation programme in uk -> residency in uk
-finish med school here -> get license here -> residency in uk

Since I have heard it is quite hard getting a residency spot in the uk, which route would be the best?

What could I do during med school that would be meriting and give me a better chance of getting in?

The residencies that I'm considering are very competitive: surgery, esp. neurosurgery. How high are my chances of matching as a FMG?

oh, I forgot: is there some exam for the uk residency, like the usmle in the us?
