Extra cirruclar and Clinical experence emergency?

I am a Canadian entering my final year of my undergraduate studies (and I'm taking a Gap year), However, my extracurriculars are lacking. Throughout my undergrad i have had problems applying myself and building a strong resume. The only research i have was volunteering in a lab over the summer and a poster presentation.

I have no volunteering experience with school clubs and organizations (is this necessary?)

For clinical experience i would like to shadow a doctor (in my last year and gap year) and i am considering taking a trip to a less fortunate country to volunteer with doctors (if available).

What I'm looking for is to apply to a SMP or a DO programs

I am wondering how to better situate myself to gain valuable ECs needed for my application.

Well you should first evaluate what clinical experience actually is. Secondly, you don't need to go to Africa to volunteer, nor do you need to volunteer with just doctors.

Medical trips are not looked at very highly. It's considered medical tourism by most. Also, shadowing is shadowing. It's not really clinical experience.
